This is self diagnosis questionnaires on your daily health condition. I took a lecture from Warren Kramer at Macrobiotic Kushi 's 2011's Summer Conference in New York and I put together here what he talked about it in that lecture. It was very interesting things he mentioned. These questions are the key to maintain your health and know how your health is doing. Is it on the right track track or getting out of the track. I will try to explain what each question meant to your health in more detail in near future, but I just want to put up this questionnaire for now.
